In today’s complex health care environment, it’s more important than ever to be informed and engaged in your own health. It is in that spirit that KentuckyOne Health Partners’ clinical team participates in the biometric health screenings offered to KentuckyOne employees and spouses as part of Catholic Health Initiatives’ Medical Plan 2016 Healthy Spirit Wellness Program.
“It is crucial to know about your current health status,” explained Amy O’Connell, APRN-BC, MSN, RN, KentuckyOne Health Partners’ interim vice president for clinical services. “Through screenings and the wellness program, we hope to engage employees in their health and connect them to a health system that can seamlessly coordinate their care needs. Our goal is to educate, empower and assist in the coordination of better health, better care and a better experience at a lower cost for those we serve.”
The biometric screenings include LDL (cholesterol), BMI (body mass index), glucose (blood sugar) and blood pressure readings. Financial incentives can be earned either for readings in the low- or moderate-risk categories or for participating in programs and activities that can lower risk.
Amy O’Connell explains that the KentuckyOne Health Partners’ clinical team can provide concierge service to expedite follow-up care for employees with an urgent health need. “By coordinating with providers participating in our clinically integrated network, we can offer a more cohesive and comprehensive interaction between the patient, provider and the clinical team. And this can lead to better health outcomes for our employees.”
Biometric screenings will be conducted at KentuckyOne Health facilities through April 29, 2016. KentuckyOne employees and spouses may learn more and register at Home.CatholicHealth.net/MyHealthySpirit.
